This Plan is sponsored by Rhodes College, 2000 North Parkway, Memphis, TN 38112.

Eligible Participants
All full-time faculty and staff categories A and B have the option to insure themselves and/or their dependents against accidental death and dismemberment, 24 hours a day, on or off the job, anywhere in the world. Part-time faculty and staff categories C and D are not eligible for this benefit.

Benefits Provided
Basic Coverage/Family Plan.
  • 100% of insured amount upon death of insured
  • 60% of insured amount -- spouse
  • 20% of insured amount -- per dependent child

There is a monthly cost for optional coverage, which would be through payroll deduction. Current rates are available in the Human Resources Office. Amounts in $10,000 increments may be purchased.

Currently the insurance company is UnumProvident Life Insurance Company of America . Request for information concerning UnumProvident accidental death & dismemberment contract terms, condition and interpretation thereof, claims thereunder, any requests for review of such claims and service of legal process may be directed in writing to:

The requirements for being covered under this Plan, the provision concerning termination of coverage, a description of the Plan benefits (including any limitations and exclusions which may result in reduction or loss of benefits) are explained in your certificate of insurance.

The plan year is July through June.
