Supplemental Group Term Life Insurance and Accidental Death and Dismemberment (AD&D)

All full-time faculty and staff categories A and B are eligible to purchase additional life insurance coverage through payroll deduction at a negotiated rate. Late enrollment is subject to the approval of UnumProvident Life Insurance Company. The coverage amount available is one times an employee's annual salary rounded to the nearest thousand, based on the covered schedule below. Rates are available in the Human Resources Office.

Employee’s Age Amount of Coverage
0 - 69 Years 100% of Coverage Amount Elected
at age 70 65% of Coverage Amount Elected
at age 75 50% of Coverage Amount Elected

Currently the insurance company is UnumProvident Life Insurance Company of America . Requests for information concerning the UnumProvident life insurance contract and its terms, condition, and interpretations thereof, claims thereunder, any requests for review of such claims, and service of legal process may be directed in writing to:
